Scodef Care stands to complete the integrated loop between the client, hospitals, GPs, Community Pharmacies and community social care.

Scodef care is owned and run by a pharmacist with a wealth of experience and knowledge in the health care sector.

At Scodef care, we use a patient-centered pathway to go the extra mile by reaching out beyond the client's personal care to help them manage their medicine more efficiently; by providing adherence support, consolidating the information, knowledge and explanation they received from their local pharmacy regarding their medications especially when there are changes made to their medicines. This will ultimately lead to: *a strong, holistic support system being made available to the patient to get the most from their medication and be in the best state of health as much as possible. * less hospital re-admission due to non-adherence and medicine errors, minimized emergency hospital visit and improved positive outcome for the client.
